This form is used for SIGNS & Symptoms of a Concussion TYBA CONCUSSION POLICY
It is the Tennessee State Law that TYBA and any/all youth sports must comply with the Tennessee Code Annotated Section 68-55-503 relative to youth sports.

This Concussion form is used to make Parents & Players aware of an Concussion -- must be signed by Player & Parent and returned to Head Coach TYBA CONCUSSION SIGNATURE FORM
Each Coach, Assistant Coach, Team Mom/Dad or anyone working with the team must fill out this form and return to the League President to be run thru the local Sheriff or Police Dept in your area. BEFORE being allowed on the field with the team. TYBA BACKGROUND APPLICATION
Each PLAYER must has one of these forms filled out by the Parents and it must be NOTARIZED and returned to the Head Coach and Coach must have with them at all times at Games, Practices, Tournaments, or events that they participate in for the safety and for Medical Attention if needed. This is a form that needs to be filled out on each player with a type of Disability - Learning, Physical, Mental and once it is filled out it MUST BE RETURNED to TYBA for them to have on file. TYBA DISABILITY REPORT
The KID - PITCH Baseball Divisions must use these Pitching Record forms, to keep record of how many Innings a pitcher pitches per game and this form must be SIGNED BY THE UMPIRE after each game and kept with you at all times, & available to be seen upon request. Failure to follow these rules can result in FORFEIT of your game or SUSPENSION OF THE COACH. Please protect the players arm. Please make sure to understand the Rules of a PITCHER and how many Innings is allowed. TYBA PITCHING TRACKING RECORD
This form is for ALL COACHES, ASSISTANT COACHES, PLAYERS, UMPIRES, & FANS, LEAGUE PRESIDENTS, SCOREKEEPERS, DUG-OUT ASSISTANTS, ETC. that has showed unprofessional conduct or poor sportsmanship during an event under TYBA League play or Tournaments, This is a FORMAL REPRIMAND and serves notice to League Presidents, Umpire In Chief, & TYBA Directors. In the incident of report being filled out it was be sent into TYBA and will be available to all parties involved. TYBA WRITTEN WARNING / DISCIPLINE NOTICE
